Schuyler County Public Health and Schuyler Hospital with partners are doing our Community Health Survey again this year with the intent of completing a new Community Health Assessment and Community Health Improvement Plan by the end of December 2019. The link to the online survey (below) will be active for the next 6-8 weeks and we encourage all to please complete it. The same survey will be offered in each of the 9 Finger Lakes Counties (Schuyler, Yates, Steuben, Chemung, Seneca, Ontario, Wayne, Livingston and Monroe) this will give us the ability to have some more in-depth information that will help us make better plans that will improve the health of our community. The coordinated effort is with all regional Hospitals and county Public Health Departments with Common Ground Health (our Regional Health Systems Agency).

 

The survey is designed to listen to our community through the stories of its residents. Your experiences and ideas will help understand and focus on our most serious health-related needs.  So we need people like you to participate in this important survey.  Participating will take about 20 minutes of your time.   But they will be 20 minutes well spent, because you will be helping us and our region —  and all its residents — young and old, rich and poor, city and suburban and rural — to get the support they need to lead healthier lives.  And at the end of the survey, if you choose to provide your name and phone number or email address, you’ll be entered into a drawing for a $500 grocery or gasoline gift card.  Can you help us please?
