Our Approach to Population Health
Population health is widely used in healthcare to describe the health status and outcomes of a group of people, rather than considering the health of one individual at a time. At Cayuga Health Partners, we are focused on improving the health and health outcomes for all the patients we serve.
Cayuga Health Partners uses healthcare data to understand and improve the health of the communities we serve. By tracking population health measures such as rates of breast cancer screening and diabetes control in different patient populations, we can understand the needs of our community and develop programs that remove barriers to health.

How Does Clinical Integration Improve Population Health?
Cayuga Health Partners uses clinical integration to break down barriers between different healthcare providers and different healthcare settings and to foster better coordination between providers. When we are successful at clinical integration, we can achieve what’s called the Quadruple Aim of health care: improved population health outcomes, improved care and patient experience, reduced provider burnout, and reduced costs through improvement.
Cayuga Health Partners is an Accountable Care Organization (ACO)
Cayuga Health Partners ACO is a clinically integrated network of primary care physicians and specialty physicians in conjunction with Cayuga Health System and its facilities who provide high quality, coordinated healthcare to our region.
What is an Accountable Care Organization?
Similar to a Clinically Integrated Network, an ACO is a group of physicians, hospitals, and other healthcare providers that all work together to provide high quality, coordinated care in partnership with Medicare. An ACO is not an insurance plan.
As a patient this means your providers are communicating and sharing information and resources with each other in the most efficient and effective way.
