Primary care providers specialize in prevention, detection, and treatment of illnesses; Internal Medicine primary care providers see adults and family medicine doctors and practitioners see all ages. At Cayuga Health’s Primary Care Centers, we strive to help our patients maintain their health through preventive care and patient education. We diagnose and take care of patients with acute illnesses, and we help patients with chronic illnesses effectively manage their disease processes.

Our Quality Data: Physician Compare

Physician Compare is a tool created by the Centers for Medicare and Medicaid Services (CMS) which serves two purposes: “to provide information to help consumers make informed decisions about their health care, and to create clear incentives for physicians to perform well.” ¹ In order to do this, Physician Compare includes general information on health care professionals and group practices to help patients find providers. Once patients locate the appropriate provider, they can compare quality data from the Physician Quality Reporting System (PQRS), Electronic Prescribing (eRx) Incentive Program, Maintenance of Certification (MOC) Program Incentive, and Million Hearts®.

Your Patient-Centered Medical Home

  • A system of care in which a team of health professionals work together to provide all of your health care needs using evidence based best practices.
  • The patient is the center of care and the most important part of the team.
  • Helps to improve communication through patient portal, electronic medical record, ability to communicate with other health care providers electronically, and care coordination when you call the office.
  • You are the center of your team.
  • Your primary care provider leads the team.
  • Nurses, medical assistants, patient care representatives, social workers, pharmacists, physical therapists, health educators, specialists, and medical facilities are also part of the team.
  • We explain your options of care and help you make decisions.
  • We help you obtain the appointments you need with us and with other offices/specialties.
  • We learn your health history and help you understand your condition or problem.
  • We are available when you need us. You can communicate with us by phone, patient portal or during an appointment. We have a provider available 24 hours per day, 7 days per week, if you have an urgent issue. If you have an emergency, please call 911.
  • We help coordinate your care. We assist in finding specialists, obtaining appointments, and sending your health history to them prior to the appointment.

WHAT YOU CAN DO:

  • Be in charge of your health.
  • Ask questions about your care.
  • Learn about your condition.
  • Follow your provider’s instructions.
  • Tell us when you don’t understand something or are upset about something.
  • Tell us if you have been to the emergency room, hospital or another provider.
  • Tell us about any changes in your condition and bring a list of all your medications/supplements to each appointment.

WHAT YOUR CARE TEAM WILL DO:

  • We will get to know you and update your records when you come in for a visit.
  • We will listen to your questions, concerns and fears.
  • We will communicate clearly with you and answer any questions you may have.
  • We will provide you with information about services in the community that may help you to learn more about your condition and stay healthy.
  • We will coordinate your care across all settings including specialists, behavioral health, hospitals, rehabilitation centers, and other facilities.
