RE: Community Health Assessment & Community Services Plan – Prioritization of Needs 2016
Dear Community Members:
As we prepare for the completion of the 2017 Community Health Assessment (CHA) and Community Service Plan (CSP) Schuyler Hospital and Schuyler County Department of Health along with the S2AY Rural Health Network, with input from several other health and human service agencies throughout Schuyler County, have started conducting a comprehensive assessment of community health needs. We have reviewed population health data statistics and engaged community members through soliciting input around local community health concerns by way of focus groups.
As a result of all the work to date, the most highly ranked health priorities that have been identified are:
- Dental
- Substance Abuse
- Obesity
- Mental Health
As we continue our assessment and begin to develop the Community Health Improvement Plan and Community Services Plan we ask for any public input related to health priorities in Schuyler County.
