Depending on your needs, there are different community resources available to help you prevent or manage conditions that last 1 year or more and require ongoing medical attention or limit your day-to-day activities.  These are considered chronic diseases and include diabetes, heart disease, cancer, chronic lung disease, stroke, asthma, Alzheimer’s, and chronic kidney disease.

1 on 1 Support 

Cayuga Center for Healthy Living: Learn how to improve your health and well-being by managing your weight, stress, and chronic health conditions. 

Cornell Cooperative Extension: Work with a nutrition educator to learn how to manage chronic health conditions with delicious, affordable, and healthy meals.  

Human Services Coalition: Learn how to fully use your health insurance and find affordable health services and prescription medicines. Get assistance with health insurance enrollment. 

YMCA of Ithaca & Tompkins County: Learn how to monitor your blood pressure at home and how to lower your blood pressure. 

Your Local Health Department: If you are pregnant, nursing, or caring for children under the age of 5, you may qualify to receive the benefits of WIC. WIC benefits include healthy food at no cost, 1:1 nutrition education, and 1:1 breastfeeding support. 

 

Group Workshops 

Cayuga Center for Healthy Living: Learn how to make changes to your life to prevent diabetes. 

Cornell Cooperative Extension: Participate in small group classes to learn how to manage chronic health conditions with delicious, affordable, and healthy meals.

Food & Fitness 

Produce Prescription Program: Ask your provider about vouchers to buy fresh fruits and vegetables at your local farmers market (at no cost). 

Tompkins County Food Resource Directory: Search this directory to find free food or prepared meals. 

YMCA of Ithaca & Tompkins County, Cortland YMCA, or Corning Family YMCA: Become a member of your nearest YMCA for access to family-friendly fitness, on-site childcare, summer camp, and swim lessons.
