Cayuga Health Partners(CHP) has been nominated for Excellus BCBS’s Circle of Excellence Award for Health Equity for our work on “Building Partnerships to Address Health-Related Social Needs and Improve Health Equity.” This award recognizes nominees who have made significant impacts on the lives of Excellus members health care and/or social services interventions, through superior examples of the Excellus BCBS mission to promote quality innovation and health equity among all patients.

  • CHP was nominated for its work building partnerships with local community based organizations to address health related social needs. CHP has made significant progress in addressing health related social needs by creating a closed loop referral process that connects patients directly to local resources in the community and shares feedback to Providers. By addressing unmet social needs we are taking steps in addressing major contributors to poor health and health outcomes for vulnerable patients in our community.
  • To date, more than 15,000 patients have been screened for social needs
  • CHP hopes to expand social needs screenings and referrals to all Primary Care locations and expand our partnerships with local community-based organizations with the ultimate goal of achieving community health integration.

Quality

At the end of June 2023, CHP released the gap list to Primary Care Physicians and our Care Coordination Team. Over 7,000 gaps were assigned to these two groups. In the next few months CHP will assign gaps to Primary Care APPs, Residents, and Specialists with Network Impacting Measures.
