
On the care management front, Cayuga Health Partners (CHP), formerly Cayuga Area Preferred (CAP) hired two additional care managers. A team of four talented nurses now actively manage several high-risk patient groups in our community.
They work closely with our PCP’s and CAP is diligently working to maximize the impact these nurses have on our patients by leveraging data we receive from various sources. On the analytics front, CAP continues to grow its ability to perform custom data analysis in support of contracting, business intelligence, and clinical quality measurement. We will be sharing performance data with practices that help us achieve our clinical and operational goals.
In the coming months, CAP will be providing extensive feedback to our primary care and specialty care offices. This feedback will center on measure specifications and what specific actions
practices need to take for CAP to be successful on measure performance. Performance on the quality measures plays a role not only in CAP’s ability to earn shared shavings through our ACO’s performance, but also determines the quality score for our network under MIPS.
