
CAP Joins Cayuga Health System In Pledge Towards Zero Suicide Mode
In June 2018 members of CAP joined nearly 50 representatives from community-based organizations at Tompkins County Suicide Prevention Coalition meeting to hear a presentation fot he Zero Suicide Model by representatives of the NYS Suicide Prevention Office. Zero Suicide surrounds a foundational belief that suicide deaths of patients engaged in the healthcare system are preventable. Simply put, Zero Suicide is a commitment to suicide prevention using a specific set of strategies and tools free to practices and providers nation-wide.
CAP, in our workings with Suicide Crisis and Prevention Services, has pledged to implement the Zero Suicide model – working, over time, to make suicide prevention a core responsibility of the Network. The journey begins with identifying key leaders to serve as part of the implementation team and performing an organizational self-study to identify opportunities to better serve our patient populations. CAP has already joined forces with Cayuga Medical Center in the pre-implementation phase and will be working towards identifying opportunities for screening and related safety planning at the practice level. Voucher Program
Supports for Health Transportation Voucher Program Training
On Tuesday July 24th, 211 representative held a training on a new transportation voucher program that will be piloted in Tompkins and surrounding counties. This program will help Medicaid recipients to access critical services recommended by their health care provider (or other service provider) but which are not currently eligible for the Medicaid transportation benefit, including assistance to get to a pharmacy, buy recommended foods after a hospital stay, or participate in classes to manage a chronic illness. CAP Ranks High in ACO Performance Results
CAP has been a Medicare Shared Savings Program Accountable Care Organization (ACO) since January 2016, and has continually provided Medicare beneficiaries in our community with low cost and high-quality, coordinated care. As an ACO, CAP brings together Cayuga Medical Center, local physicians, and other health care providers in our community to work together to provide higher-quality coordinated care across the health care delivery spectrum to our patients, while also working to slow the growth of health care costs.
In August, the Centers for Medicare & Medicaid Services (CMS) released the 2017 Shared Savings Program ACO Performance Results. Of all the ACOs in New York State, CAP achieved among the highest quality performance score of any ACO in New York being measured on performance; surpassing 33 other ACOs in the State. CAP achieved a 95.56% quality performance score for 2017, this high-quality performance demonstrates the value of care that is provided in our community.
CAP not only achieved a high-quality performance score but also had the lowest yearly per Medicare beneficiary spend of all ACOs in New York State. Nationally, CAP ranks as the 5th lowest cost ACO out of 480 ACOs across the United States, with only 4 other ACOs in the Nation having a lower yearly per Medicare beneficiary spend in 2017
CAP Care Management Partners with Excellus to Outreach ‘Rising Risk’ Members Excellus notes exceptional results seen in the early stages of partnership with CAP Care Management
Earlier this year Excellus launched a new program in efforts to support partnering Accountable Cost and Quality Arrangements (ACQAs) by using health plan resources to engage members who may be, what Excellus refers to as, “lost to care”. CAP was quick to take interest in the project and, with an outreach engagement rate of 92%, was confident engagement would be more effective, and efficient, if CAP Care Management deployed the resources to do similar work. After careful planning in collaboration with our partners at Excellus and some initial groundwork by Excellus care managers, CAP took the lead in outreaching the this ‘Rising Risk’ population
Results: In the initial three days of including this work into their workflow, CAP Care Coordinators were able to reach 61% of the members identified. In addition, 36% of Rising Risk members were identified as having completed a PCP visit, or scheduled for one, and 53% of care gaps were identified as closed or scheduled to be closed
In a subsequent partnership ACQA work group meeting with Excellus, it was mutually decided that CAP’s Care Management team is best suited (than previously-deployed Excellus resources) to perform outreach on the network attributed Rising Risk Population. CAP anticipates continued success in this, and like programs, moving forward and hopes this will be the first of many areas in which we partner with Excellus and gain continued support of CAP Care Management Activities.
