CAP has partnered with HealtheConnections (HeC), the RHIO for central New York, based in Syracuse, to aggregate clinical data from each CAP practice into a central database. CAP will leverage this data to achieve population health objectives.


As CAP moves forward with its IT and data warehouse capabilities and we expand our engagement in Value Based Contracts, developing clinical   data connectivity with practices has become a high priority. Currently, the majority of CAP quality metrics are reported via PQRS codes added to your billing information in claims data.  While this method has generated reliable data for quality and population health initiatives, the addition and utilization of clinical data feeds will allow CAP to develop more granular quality measures and more accurate reports. Moreover, clinical data feeds from your EMR provide a more direct view of what is documented in your EMR than the claims method. We have all observed that the billing method of coding sometimes does not reflect the care you are delivering or document in your EMR.
