CMA PCP Offices are successfully on the Network Access Team’s call lines. Our mission for taking on these practices is to improve access for patients receiving and navigating care as well as providing a more centralized location for patients to access care.


Care Coordination

Care Coordinator, Brittany Thorpe coordinated with Optum Rx to eliminate >$30 copay for pediatric patient’s routine injectable medication. Last week, Rx was delivered to patients’ home without a payment required!


VBP Successes

In support of our VBP Contracts, CHP creates a Gap List that includes patients not meeting measure criteria. Each Care Coordinator is working to Close these Gaps in Care with a focus on hypertension patients to improve network performance on Controlling High Blood Pressure. Patients with COPD often have multiple hospital readmissions, with this being said, the Care Coordination Team is following-up on patients admitted to CMC to reduce these readmission rates.


Quality

Distributing Q3 Report Cards, including APPs for the first time. CHP Report Cards contain performance data for individual, practice, and network level performance.
