CHP Clinical Integration Committee

The CHP Clinical Integration (CI) Committee is responsible for the oversight of:

• The business operations
• The performance initiatives of the CI program including the infrastructural and support of the CI program and as a Physician Hospital Organization
• The sustainability of the health system and network.

CHP Contracts and Finance Committee

The CHP Contracts and Finance Committee provides oversight for:

• Payer participation, contract evaluation, and relationship management
• Provider contracts
• Monitor financial performance of clinical integration contracts

The CHP IT and Data Governance Committee provides oversight for:

• CHP’s health information technology that allows for the exchange of patient treatment and medical management information as it relates to clinical integration
• Aggregate data collection of metrics and initiatives as required for reporting and benchmarking for clinical integration
• Promoting information sharing and interoperability

• Development of the provider incentive plan
