Cayuga Health Partners is the clinically integrated network affiliated with Cayuga Health System. We support a network of healthcare providers who work closely with each other to facilitate the improvement and coordination of patient care, and share responsibility across different settings.

Community Partnership & Community Support

Being healthy shouldn’t be hard. When health care and community-based organizations (CBOs) work together, you thrive. For over 10 years, we have built relationships with our local CBOs. The result is a network of health care providers and community-based organizations (CBOs) that work together to address everything that shapes your health. Having enough food, a safe place to live, and a way to get to your appointment all play a role in your health.


What does the network do?

The goal of the network is to connect you to the services you need to live the healthiest life possible. To achieve this, health care providers and community-based organizations (CBOs) need to talk to each other and work together, so everyone is fully informed, and you get the resources you need to thrive.


How will I benefit from this?

To talk to each other and work together, health care providers and CBOs need your permission. By consenting to share your information with this network you can:

  • Save time being asked to repeat basic information when you get referred from one place to another
  • Get connected to community resources faster
  • Allow different care workers to communicate with each other about your needs quickly and efficiently.

What types of information will be shared and why?

  • Contact information: So that we can follow up with you by phone, email, text, or however you want to be contacted. This includes your phone number and email.
  • Health-related social needs: So that we can connect you to the right community resources. For example, food, housing, or transportation assistance.
  • Basic demographics: So that we can suggest services that you might be eligible for. For example, your age and the county where you live.
  • Previous referrals & enrollments: So that we have information about what other services you may have tried and can avoid referring you again to something that might not be a good fit.

How can I get connected to community resources?

Just let your doctor, nurse, or any other provider know that you want to be connected to community resources. They will create a referral on your behalf. If your health care provider’s office is not a part of the community health network, call 211.  211 has operators available 24/7 who can direct you to local community resources.


What happens after my provider makes a referral?

Our goal is for no patient to fall through the cracks.

  1. A local organization that provides navigation and referral services will receive your information and will confirm with your provider that they received it.
  2. Their staff will call, email or text (whichever you prefer) to learn more about you and the kind of support you need and want.
  3. Based on what they learn from you, they will work with you 1:1 to find the right resources to meet your needs and will follow up with you to see how things are going unless you decide you no longer want their assistance.
  4. Once you have had your last follow-up conversation, they will send notes back to your provider to let them know which programs or services they were able to connect you with, and whether or not your needs were met.
