As a patient, you will be asked questions about yourself during your visit. You will be asked about your race, ethnic background, sexual orientation, sex, gender identity, and preferred language. This information helps us provide better care for our patients by allowing us to understand your needs.

We care about making sure everyone we see gets high-quality care and we want to improve health care for everyone we serve. This information also helps us identify and address health care disparities that exist in our community and allows us to treat each patient with respect, use your chosen name and pronouns correctly, and provide care with dignity.

Your answers are kept private and are protected by law. What you share is always up to you! We ask because we care, you decide what to share.

Your provider will use this data to better understand your health needs and give you the best care. The data will also be used to find and address health disparities. When we look at reports about health disparities, your name and other identifying information will never be attached to the report.

The unequal differences in health status or outcomes across different groups of people. Some groups of people are at a higher risk of getting sick and having worse outcomes when they do get sick. The data you provide us helps us stop health disparities.

You can always choose not to answer these questions. On the form you are given, there is a “Prefer not to answer” choice; you can check that option if you do not want to share this information. If you change your mind later, we can add this information another time, or you can update your patient portal at any time.

You only have to answer the questions that you want to.

Your information is private and protected, so there are only a small number of people who see this information. The person who puts the data into the computer, the person who takes you to the room, and your health care provider. No matter who sees your answers, your information is confidential and protected by law.
